This form is to be filled out by parents. As stated on your course registration card, class change requests and team requests were due by April 15th. 

Schedule adjustments will only be made if your student:

• is missing a class 
• is in need of an intervention class
• have classes that are cross-teamed 
• other Skyward error

 If you feel that there is an error on your schedule or other concerns, we will happy look into it and get back to you. 

For Educational Release requests (late start), you will need to fill out the district form at releases.canyonsdistrict.org as well as this google form.

• Due to the late request for “late start”, you may not be able to keep your original course requests. 
• Counselors reserve the right to drop any elective classes to accommodate the request
• Per Canyons School District policy, Educational Release requests will not be accepted after August 26th

School counselors will work on the correction list in order that we receive them. Thank you for your patience and understanding. 

Any changes made to your schedule may effect other classes.  All changes are final.
